Yes. There is a special form for amending your original. Look in your instruction booklet and you will find it.

You need to file an amended tax return, a 1040X. It is only 2 pages, and you must attach the pages that change – that are different from your original return.

Yes, assuming that you need to correct your personal return, you will need to file a Form 1040X. It is available for download from the IRS site. See below.
