I applied for an IRS ITIN for my wife over a year ago, but never received a response?

Application W-7 was filed, as is the process, with my 2004 federal tax return. No response of any kind from IRS.

Should I just continue to write “ITIN requested” on all IRS forms that request a SS/ITIN for my wife until she gets a SS#?
Is it really a requirement to hire an accountant in order to request and receive an IRS ITIN?
