No you can use a subsitute w-2 form.

yes. you have to report your income and it is reported on a w2. the only way you wouldnt have a w2 is if you sub contracted and got a 1099 or if you own your own business in either of those cased though you would not file ez but file form 1040a so since you are not filing that i know you have a w2 and you have to report it. even if you do a substitute w2 you will still have to report all the income and taxes off of it and you will need the ein number of the employer.
