Use 1040x. You have 3 years from the year in question to correct any mistakes. You fill out the amended return just as you did your return from 2 years ago with the exception of the corrected error (that is, the IRS will compare both returns and find the error themselves).

1040x is right, but the part about 3 years is questionable. the IRS has 3 years for assessment, and 6 years if the error is a substantial understatement (error>25% of gross income).
