check your pay stubs. name of employer should be there. check your cell phone. look under work in your list of contacts. call that number. ask for your W-2. They should have sent it to you already. companies need to send W-2s to its employees (former & present) by jan. 31.
For Fed W-2s you use form 4560 or 4560T. I suspect it’s similar for states. Call your state Dept of Revenue.
FYI-info you get from filing form 4560 does NOT include state info.
Remeber where you left the pipe and figure out who you worked for. How do you forget who you worked for in the past year. To get your W-2 I would first stop doing whatever it is that is making you forget.
