1099-R is for rolling over money from one 401k to another. If you just started a new 401k and didn’t roll anything over from a previous one, you will not get a 1099-R, so just enter a zero. If you did rollover from a previous 401k and lost your 1099-R, then you need to contact that fund company and ask that they send you a new 1099-R.

You only get a 1099-R when you take money OUT of 1 401K, not when you put it in.
